The construction of the church was begun in the 14th century (1339), for the Augustinian monastic order, and was concluded in 1345. It was finally completed in 1475 when it was consecrated. In the 16th century (between 1553 and 1559) it was radically transformed by the addition of vaults in masonry.
The façade follows the classical canons of Gothic monastic architecture in Lombardy and has the typical high façade in bricks, divided in three by buttresses and surmounted by a loggia. The bell tower is later (1461) and is lightened by elegant mullioned windows. The large interior (71.9 metres long and 21 metres wide) follows a basilica plan with the central nave covered by a barrel vault. There are elaborate frescoes and several notable pictorial works including: in the Cavalcabò chapel (third on the right) a cycle of Marian frescoes in a late Gothic style attributed to Bonifacio Bembo. Also noteworthy are portraits of Bianca Maria Visconti and Francesco Sforza. In the fifth altar on the right is an altar piece by Perugino depicting "The Madonna Enthroned with Saints James and Augustine" (1494).
